REQUESTED MOTION/ACTION

Recommended Action:
a) Approve Amendment #3, of Contract #16-322, Self-Insured Vision Insurance with EyeMed Vision Care LLC, of Mason, Ohio, to renew the annual contract from the term October 1, 2024, through and including September 30, 2025. This is for self-insured vision insurance services; and
b) Authorize the Chairman, or his designee, to sign the Third Amendment to Fee for Service Agreement.

Budgeted Action:
No action needed. Budgeted in the Risk Management Fund - Health Insurance Budget to be approved in the FY2025 budget process. Funding for this expenditure comes from Insurance premiums.

BACKGROUND (Why is this Action Necessary, and What Action will be accomplished)

Background:
This is an annual contract with EyeMed Vision Care, LLC, to provide self-insured vision insurance services for the Board of County Commissioners, Clerk of Circuit Court, Property Appraiser, Tax Collector, Supervisor of Elections, Charlotte County Airport Authority, Peace River Manasota Regional Water Supply Authority, and Court Administration.
This Amendment allows for the renewal of current contract as awarded for an additional year from October 1, 2024, through and including, September 30, 2025, with the same plan design and rates at no additional cost. The appointed Agent of Record for the County is The Gehring Group. A copy of the Scheduled of Benefits is attached.
